How do I sign up to cover events? |

Download the affiliation agreement and the Covenant
not to compete, fill those out and return it with a copy of your
license, insurance, CPR card and registration fee. |

What does the registration fee cover? |

This yearly fee helps cover the cost of the website. |

Do I get anything for referring a new client or

Your next year’s affiliation fee will be
waived. |

What if I need supplies? |

If equipment is not supplied at the event, turn
in a list of supplies used and you will be restocked. |

How do I get paid? |

Submit the hours you worked on an event. For events
covered from the 1st –15th you will be paid on the 30th.
For events covered from the 16-31 you will be paid on the 15th
of the following month. |

Are you taking taxes out of my paycheck? |

No. A 1099 will be submitted at the end of the
year. |
